What is a D4 medical?

The D4 medical is an examination that consists of an eyesight test using a snellen chart, blood pressure checks to ensure your BP meets the given criteria, and a questionnaire regarding your medical history. All to ensure you meet the DVLA group 2 standard.

What does the medical examination involve?
The examination will only take around 30 minutes. It includes an eye test, blood pressure check, physical examination and BMI check. You will be asked questions about the following topics:

- Cardiovascular system including high Blood pressure.
- Endocrine system such as diabetes
- Musculoskeletal such as back pain.
- Neurological such as stroke
- Psychiatric such as depression
- Alcohol and drug history
- Vision

Please bring with you:
1 - ID (ie. passport or driving licence)
2 - A copy of the summary of your GP records
3 -  Driving glasses/contact lenses & Prescription (If applicable)
3 - D4 medical form - Download Here - Please fill in the grey section on page 1 (left hand side) ‘Your details’ and page 8 (authorisation and declaration) along with your name and date of birth at the foot of all other pages on the D4 form

The medical is carried out at GP Matters (24 Buckingham Terrace, Glasgow G12 8ED).

Fees
D4 / DVLA Medical - £150
Please note that in the event that your application is not successful we cannot refund the cost of your D4 / DVLA Medical.
