Procedure:
The examination will only take around 30 minutes, it includes an eye test, blood pressure check, physical examination and BMI Check. The Doctor will ask various questions about your vision, heart, diabetes, mobility, mental health and neurological functioning.
The doctor will complete the form based on your answers, medical history, medications and the results of the examination findings.
Please bring with you:
- ID (ie. passport or driving licence)
- A copy of the summary of your GP records
- D4 medical form -

ill in the grey section on page 1 (left hand side) ‘Your details’ and page 8 (authorisation and declaration) along with your name and date of birth at the foot of all other pages on the D4 form- Your Driving glasses/contact lenses & Prescription (If applicable)
Location:
The medical is carried out at GP Matters (24 Buckingham Terrace, Glasgow G12 8ED).
Fees
D4 / DVLA Medical - £150
Please note that in the event that your application is not successful we cannot refund the cost of your D4 / DVLA Medical.
