Procedure:
The D4 medical is an examination that consists of an eyesight test using a snellen chart, blood pressure checks to ensure your BP meets the given criteria, and a questionnaire regarding your medical history. All to ensure you meet the DVLA group 2 standard.
The doctor will complete the form based on your answers, medical history, medications and the results of the examination findings.
We ask drivers to book an optician/optometrist appointment (to complete the vision assessment) before attending the medical with us.
Please bring with you:
- ID (ie. passport or driving licence)
- A copy of the summary of your GP records
- D4 medical form -

ill in the grey section on page 1 (left hand side) ‘Your details’ and page 8 (authorisation and declaration) along with your name and date of birth at the foot of all other pages on the D4 form- Vision assessment (page 2) completed by an optician/optometrist

The medical is carried out at GP Matters (24 Buckingham Terrace, Glasgow G12 8ED).
